Before Surgery
LIFETREE PAIN CLINIC will contact you prior to surgery. Please bring your questions and problems to our attention during the first call.
Follow these guidelines the day before your surgery to ensure your operation goes smoothly:

  • Please call us the business day before surgery between 10 a.m. and 2 p.m. to confirm your arrival time and to allow us to give you preoperative instructions. It is critical that you follow these instructions precisely.

  • Do not eat or drink anything after midnight the night before surgery.

  • Arrange for a responsible adult to remain in the facility during your surgery. This person will need to drive you home and stay with you following surgery. You will not be allowed to leave the clinic alone after your surgery. You may feel dizzy and/or drowsy for several hours

  • If there is a possibility you are pregnant, please let us know and contact your obstetrician.

  • Tell us if you have been taking medication, have a serious illness, have been exposed to a communicable disease or have a cold, sore throat, fever or acute illness.

  • Your insurance and payment information must be obtained before surgery to confirm benefits. If your insurance will not cover all charges, we will provide you with an estimate of the balance.

Day of Surgery
To help us meet all your needs, please follow these guidelines:

  • Please arrive at the clinic as instructed by our staff (approximately 30 minutes prior to surgery). Your punctual arrival is necessary to prepare you for your procedure.

  • Wear comfortable clothing that can be folded and put in your dressing room locker. Leave all jewelry and valuables at home.

  • Do not wear fingernail polish or make-up, especially eye make-up. Eye injury can result from mascara flakes getting into your eyes.

  • If you wear contact lenses, consider wearing your eyeglasses. If you do not have glasses, bring your contact lens storage container and solutions.

  • You will be asked to sign a surgical consent form. A parent or legal guardian must sign for a minor.

    Patients may be accompanied by one visitor. Please leave children at home.

After Surgery

  • You should go directly home and rest the remainder of the day. If you had anesthesia, have someone stay with you the first night. Do not drive, make any important decisions or use power tools or appliances for at least 24 hours after your surgery.

  • You may be required to stay at the clinic for up to three hours after surgery. Amore accurate time can be given at admission. Payment/Credit Policy LIFETREE PAIN CLINIC will verify your insurance coverage and eligibility prior to surgery. Payment may be deferred if your insurance company verifies coverage and eligibility prior to surgery. However, any deductible and/or co-payment will be due upon admittance to the clinic on the day of surgery. Should any charges be rejected by insurance, full payment is due from the responsible party within 30 days. Money orders, cashier's checks, Visa, MasterCard, Discover Card and American Express are accepted. If you are paying by cashier's check, two checks will be required: one for the clinic and one to cover anesthesia. Charges for anesthesia and non-insurance cases are to be paid in full at the time of service. An exact quote for many procedures can be given. Bring your insurance card with you. If your insurance company requires a claim form, please bring two with you.

Other Policies

  • If you have not previously been seen in the clinic, a nurse practitioner will perform a consultation.

  • Each patient receives a pre-operative evaluation by a member of the anesthesiology team.

  • LIFETREE PAIN CLINIC is a smoke-free facility in compliance with the Utah Clean Air Act.

  • In consideration of patients awaiting surgery, no food or drink is permitted in the clinic.

How to Become a Patient

A new patient needs a referral. After we receive your medical records, we will mail you a new patient packet. You return these completed forms to us along with a reservation fee that is refundable on your first visit. We will then call you to set up an appointment.
